- Adirondack Rural Health Network (ARHN)
A multi-stakeholder, regional coalition that informs on planning assessment, and provides education and training to further the New York Department of Health Prevention Agenda.
- Enrollment Assistance Services and Education (EASE)
Offers health insurance enrollment assistance and community awareness services for the Federal Affordable Care Act implementation in New York State.
- Population Health Improvement Program (PHIP)
Provides resources and infrastructure to promote the state’s Triple Aim of improving population health, bettering the quality of the patient experience, and lowering health care costs.
- Prevention and Education Initiatives
Provides prevention education in Washington County.
- AHI Health Home
Provides enhanced coordination of medical and behavioral health care by linking community and social supports with health care for high-risk Medicaid members.
- Adirondack Medical Home Initiative (AMHI)
Collaborative effort by health care providers and public and private insurers to emphasize preventive care, enhanced management of chronic conditions, and the assurance of a close relationship between patients and their primary care providers.
- AHI PPS: Delivery System Reform Incentive Payment (DSRIP) Program
A Performing Provider System (PPS) is a network of providers working together to achieve common goals including reducing avoidable hospital visits by 25 percent over the course of five years.
