More Than $1.2 Million Awarded to AHI’s Health Home

Health Home Development Funds in the amount of $1.227 million were awarded to AHI’s Health Home. Care Management agencies will be involved in determining which of the following initiatives (one or more) will be funded: member engagement/promotion; workforce training/retraining; health information technology and clinical connectivity; joint governance technical assistance.

Adirondack Medical Home Initiative Recognized Nationally

The Adirondack Medical Home Initiative (AMHI) received an extension by Centers for Medicare & Medicaid funding. This AHI program is one of five Multi-payor Advanced Primary Care Practices (MAPCP) demonstrations nationwide to receive an extension.

ARHN Develops Community Needs Assessment

The Adirondack Rural Health Network (ARHN) partnered with the AHI Performing Provider System (PPS) Delivery System Reform Incentive Payment (DSRIP) Program planning team to do an in-depth Community Needs Assessment (CNA). The CNA is a comprehensive assessment of current health care resources, available community-based services, demographic information, health needs of the population, and health care service use data to aid the PPS in choosing appropriate projects to improve the health of the community.

EASE and Community Partners Help 6,000 Obtain Health Insurance

AHI’s Enrollment Assistance Services and Education (EASE) program and community partners were successful in helping educate and assist nearly 6,000 regional individuals, families and small business owners enroll in affordable health insurance coverage through the NY State of Health Marketplace.

The EASE program has offered free health insurance enrollment assistance services in Clinton, Essex, Franklin, Hamilton, Saratoga, Warren, and Washington counties since the Marketplace opened on October 1, 2013.

AHI PPS Receives DSRIP Project Plan Award

AHI and its Performing Provider System (PPS) partners have the opportunity to earn up to $186,715,496 in performance-based payments from the New York State Department of Health (DOH) under the Delivery System Reform Incentive Payment (DSRIP) Program’s Project Plan. Funding will be used to increase the level of health care service integration necessary to better manage population health and embrace payment reform in nine Upstate New York counties. The AHI PPS is one of 25 awardees in the state.

Population Health Improvement Program Offers Resource Web Portal

The North Country Population Health Improvement Program (NC PHIP) offers a resource portal website, HealthyADK, with the goal of advancing health in six North Country counties. The site provides up-to-date community health data, a report creation feature, and links to evidence-based programs and resources.
