AHI: Lead, Empower, Innovate
Our motto at AHI is “Lead, Empower, Innovate.” Our success and sustainability is contingent on the strength of relationships with diverse stakeholders, as well as the capacity to effectively and efficiently communicate and collaborate regionally. We rely on our member organizations, Adirondack Health, Glens Falls Hospital, Hudson Headwaters Health Network, St. Lawrence Health System, and The University of Vermont Health Network – Champlain Valley Physicians Hospital, to advance patient, provider and community connections.
AHI programs benefit from the participation of nearly 200 committee members from more than 100 organizations representing a broad range of health, community, and business sectors.
Initiatives, including the Delivery System Reform Incentive Payment (DSRIP) Program and Population Health Improvement Program (PHIP), contribute to AHI’s expansion and our role as a leader and innovator for change in our communities.
Our programs include:
- Adirondack Health Home
- Adirondack Medical Home Initiative – AMHI
- Adirondack Rural Health Network – ARHN
- AHI PPS – DSRIP
- Enrollment Assistance Services and Education – EASE
- Population Health Improvement Program – PHIP
- Prevention and Education Initiatives
Our capacity for services has grown, along with our experience and expertise, enabling growth and transformation.
