Identifying areas of need

We took a long look at our community including its demographics, health services delivery structure, health status, health services utilization patterns, and its assets and resources, and were able to identify areas of need.

Conclusions from the CHNA:

  • Mental health conditions affect over a third of the population across almost all counties.
  • Tobacco, alcohol and substance abuse are prevalent and drives significant Emergency Department utilization across the region.
  • Access to primary care is a significant need across the region both in terms of general access, as well as providing a setting where proper chronic disease and prevention-focused treatment can be delivered.

The needs identified underlie the DSRIP projects we selected for implementation.
