Health System Transformation
- AHI Health Home
Provides enhanced coordination of medical and behavioral health care by linking community and social supports with health care for high-risk Medicaid members.
- Adirondack Medical Home Initiative (AMHI)
Collaborative effort by health care providers and public and private insurers to emphasize preventive care, enhanced management of chronic conditions, and the assurance of a close relationship between patients and their primary care providers.
- AHI PPS: Delivery System Reform Incentive Payment (DSRIP)
A Performing Provider System (PPS) is a network of providers working together to achieve common goals, including reducing avoidable hospital visits by 25 percent over the course of five years.
